Medical Director
Dr. Michał Pawlik, MD, CCFP, FCFP, is a family physician and the Medical Director of the Saanich Peninsula Outreach Team (SPOT-SDELḴ). Born in Poland, he earned his medical degree from Jagiellonian University in Kraków in 2003 before moving to Canada, where he completed his certification in Family Practice at the University of Manitoba in 2007.
Dr. Pawlik has a particular interest in mental health and addiction medicine alongside general family practice. He is dedicated to improving healthcare access for high-needs patients on the Saanich Peninsula. Through SPOT-SDELḴ, he is helping develop a low-barrier health centre and mobile outreach service to make care more accessible.
Outside of work, he enjoys spending time outdoors, hiking, kayaking, and camping.
Family Physician
Dr. Kate Evans is a mother of 3 and has been a family physician for over 20 years. Of British and Irish ancestry, Kate earned her MD from McMaster University and completed her family medicine residency at Dalhousie.
Kate works with both SPOT and Cool Aid outreach, which has strengthened her awareness of the connections between intergenerational trauma and both physical and mental health. She played a pivotal role in establishing the Saanich Peninsula Youth Clinic and was honoured with a Sidney Rotary Club award in 2018 for her contributions. She has also served on the board of the Division of Family Practice and co-chaired the local action team of the Child and Youth Mental Health and Substance Use Collaborative.
Outside of work she enjoys exploring the beautiful W’SANEC territory with her family, meditating, riding her e-bike, and making art.
Medical Office Assistant
Jasmine Kosmuk is a dedicated Medical Office Assistant at the Saanich Peninsula Outreach Team (SPOT-SDELḴ) Clinic, where she provides administrative and patient support to ensure a welcoming and efficient healthcare experience. With honors in her Medical Office Assistant program and hands-on experience in medical settings, she is passionate about delivering organized, compassionate care to every patient who walks through the clinic’s doors.
Her background includes working in fast-paced clinical environments, assisting physicians, managing medical records, and coordinating appointments with attention to detail and professionalism. She thrives on creating a supportive atmosphere where patients feel heard and valued.
Outside of work, Jasmine enjoys spending time with her family, gardening, camping, and fishing, which helps her maintain a balanced and positive approach to her role at SPOT-SDELḴ.
Registered Nurse
The SPOT clinic is privileged to have access to RN Miriam Breton, the Nurse Lead for the Saanich First Nations Adult Care Society (SFNACS).
She collaborates closely with SPOT-SDELḴ Outreach Society doctors to provide compassionate, patient-centered healthcare. With 35 years of nursing experience, primarily in community homecare, she is dedicated to advocating for and supporting the well-being of all clients. In her free time, she is passionate about learning about the Coast Salish communities and finds joy in walking along the ocean, collecting sea glass, and appreciating the beauty of the coastal landscape.
